I've got a pair of civic headlights here that I bought for a conversion on my CRX, but decided to go with a different conversion. They are brand new, and have never been mounted. They have been sitting in my room for the past year. They come with the bulbs and this credit card-like thing that states that they are SAE/DOT compliant.

I don't have access to a camera, so I ripped a couple pictures from anyother guy with the same headlights mounted on his civic:
